C4YH at Home app for iPhone and iPad

Patients can track the care team in real-time when en route and get notified of any delays or changes in the route.The app enables patients to access their appointment.For the Care Team, scheduling appointments by geographical proximity with the flexibility to modify the route is available. It allows for efficient routing with Google Maps
and voice directions. The team can communicate with the patient via this app. It also provides for mileage tracking.
